Service Provision and limitations 1.Clients must understand that nutritional dietary therapies require time, patience and consistency; nutritional healing takes time and results will vary between individuals. However, the more attention and time spent on making the suggested changes, the better the results will be. 2. Nutritional therapies are not a 'short term' answer, 'overnight fix', 'crash diet' or designed with only one goal in mind (weight loss); These therapies aim to improve ALL aspects of health and wellbeing as well as improve weight loss outcomes. Whilst success rates are high in those who demonstrate adhesion to plan and consistency, dietary plans are not a "MAGIC BULLET" or "MAGIC PILL", therefore overnight results are unrealistic. 3. All dietary suggestions are aimed at improving health and wellbeing, with extreme care and caution being placed on avoiding any possible food vs medication interactions. It is the client responsibility to disclose all medications and treatments to avoid any potential contraindications; failure to disclose, and any subsequent (although highly unlikely)contraindications will be at the fault of the client. All effort will be made to question and re-check client details to avoid any such omissions. 4. All dietary advice is for guidance but should not take the place of the advice of medical professional. Please ALWAYS consult your doctor if you have any concerns in changes to your dietary or lifestyle routine. 5. CR NUTRITION does NOT, in any way, promote the cessations of prescribed medications. If dietary modifications lead to the improvement of symptoms, it is the client responsibility to liaise with their prescribing practitioner to discuss medication adjustments. 6. All dietary supplements; Vitamins, minerals and inclusions are of a suggestive/advisory nature and are not prescriptive. 7. CR NUTRITION DOES NOT, in any way, promise, suggest, guarantee or direct any client to the assumption or idea of complete healing. Results are variable, however all possible effort will be made to promote best possible outcomes for the client on a case by case basis. 8.CR NUTRITION reserves the right to refuse service for any reason deemed a conflict of interest or ethically impractical. 9. All client plans, information, texts and literature will remain the intellectual property of Carrie Ross - CR NUTRITION & BARIATRIC HEALTH COACHING protected by both Australian and US Copyright Law; and are not to be copied, shared or reproduced for any purpose.
Client Pre-Appointment Questionnaires 1. For ALL Holistic based assessments clients will be required to complete at least one pre-appointment questionnaire. 2. Clients will be granted access after purchase to complete the pre-appointment questionnaire, and given instructions via email. 3. Consultations will be booked an average of 5 business days after the receipt of completed and returned questionnaire data. 4. It is the clients responsibility to provide complete and concise information on all aspects of their health and wellbeing; as Holistic plans will be created based on the information provided. Failure to provide complete information prior to plan creation and consultation; and subsequent omission from the treatment plan will be at the fault of the client. 5. Plans can only be created using the information provided, therefore more information will translate to a better plan. 6. Clients MUST Be specific and take time with their answers; especially with regards to meal plan questionnaires. Failure to disclose specific food aversions and having them subsequently included in meal plans will be at the fault of the client. Carrie will endeavour to provide suggestions for food exchanges where possible, however, meal plans will not be replaced. 7. No changes will be made to plans after this time has lapsed.
12 Week Coaching Plans, Prepaid Services and Follow-up Appointments . 1. As outlined in product description, you have paid for access to a coaching service via your selected format. Access to coaching is available for the duration of the 12 weeks, however it is the client 'responsibility to make contact and check in when required to derive best value from the coaching resource. 2.Email Coaching and Skype Coaching sessions do not accrue, If you do not check in that week, it will be a session lost. Client's will not be 'chased down' to follow-up or check in, therefore failure to contact CR Nutrition on any or all of the weeks consecutive to agreed initial consult/plan delivery, will be considered as declination and forfeiture of this service, however opportunity to access this service will remain open and accessible for the entire 12 week duration regardless of failure to contact, but no longer than the 12 weeks. 3. Failure to utilise this service does not constitute reason for refund, as services were accessible for the 12 weeks. 4. Your access to 12 weeks coaching expires on the 12th week after your Initial consultation, unless otherwise negotiated with your consultant. 5. Prepaid Follow-up appointments included in package plans must be made by contacting CR Nutrition within the 12 week period. Clients again will not be 'chased down' to make their appointments. Failure to book your follow-up appointment within an additional 12-weeks after the completion of the initial 12week period will be considered a declination and forfeiture of this service, and a full-fee follow up appointment will be required if client wishes to revisit their plan. 6.PREPAID BLOCK SKYPE SESSIONS other than those included in Skype option 12-week pack- have an expiry of 6 months after the purchase date. 7. Email Coaching blocks will begin within 24 hours of purchase and expire within 1-Calendar month of purchase. 8. Add on meal plans will be provided to client within 1 week of purchase, 9. Consultation bookings will be made with no less than 1 week between receipt of questionnaire data and consult.
Appointment ‘NO SHOW’/ Failure to attend live consult bookings. FAILURE to attend your appointment, or late arrival by more than 15 Minutes will be considered a "NO SHOW" , no show appointments can be rescheduled, however more than after 2 consecutive "NO SHOW" for the same booking, cancellation will occur and monies will be forfeit.
